Complete the excel spreadsheet questionnaire. If something is not clear, please highlight the question in yellow so that we can go over the unclear question(s) together and continue answering the questionnaire by moving on to the next question. Once you have answered all of the questions, please email or upload (on the right) your answers.
Step 2: Complete additional schedules (if applicable)
Not all of the above schedules will apply.
Step 3: Send us your documents.
Please upload your documents using the below button. Please send the administrator username and password to any Quickbooks, Sage 50, files you upload.
